Between October 25, 2022, and November 25, 2022, FORMS-H application packages will be added to active funding opportunity announcements. The <a href=\"https:\/\/grants.nih.gov\/grants\/how-to-apply-application-guide\/forms-h\/general\/g.100-how-to-use-the-application-instructions.htm\" target=\"_blank\" rel=\"noopener\">How to Apply \u2013 Application Guide<\/a> was updated on October 25 with FORMS-H application form instructions to prepare for the transition. Also see Guide Notice <a href=\"https:\/\/grants.nih.gov\/grants\/guide\/notice-files\/NOT-OD-23-012.html\" target=\"_blank\" rel=\"noopener\">NOT-OD-23-012<\/a>.<br \/>\nAll form changes are listed in<a href=\"https:\/\/grants.nih.gov\/grants\/electronicreceipt\/files\/high-level-form-change-summary-FORMS-H.pdf\" target=\"_blank\" rel=\"noopener\"> High-level Grant Application Form Change Summary: FORMS-H<\/a>.<br \/>\nA key change in FORMS-H is support for the implementation of the<a href=\"https:\/\/sharing.nih.gov\/data-management-and-sharing-policy\" target=\"_blank\" rel=\"noopener\"> 2023 NIH Data Management and Sharing Policy<\/a>. A new <strong>Other Plan(s)<\/strong> attachment field (see Figure 1 below) has been added on four application forms, where applicants can attach the required Data Management and Sharing Plan if required by the Funding Opportunity Announcement (FOA).<\/p>\n<ul>\n<li>PHS 398 Research Plan<\/li>\n<li>PHS 398 Career Development Award Supplemental Form<\/li>\n<li>PHS 398 Research Training Program Plan<\/li>\n<li>PHS Fellowship Supplemental Form<\/li>\n<\/ul>\n<p>The intended due date for your application determines the correct form package to use.<\/p>\n<ul>\n<li>Application due dates <strong>on or before January 24<\/strong>, 2023: <strong>FORMS-G<\/strong><\/li>\n<li>Application due dates <strong>on or after January 25<\/strong>, 2023: <strong>FORMS-H<\/strong><\/li>\n<\/ul>\n<p>For work in progress applications prepared using FORMS-G that need to be moved to FORMS-H for a due date on or after January 25, 2023, authorized users can use the copy feature in ASSIST to move the information in the existing application package to a new one. Grants.gov Workspace and many institutions\u2019 system-to-system solutions have similar features.
