Integrated Care team presents at national SAMHSA conference<\/strong><\/p>\n Angela Clay, program coordinator, Princess Douglas-Williams, nurse care manager, and Robin Reed, MD, MPH, director, Integrated Care, presented \u201cPopulation health\/patient centered medical home elements\u201d on a panel at the National SAMHSA Conference in Austin, Texas in August 2017. The presentation looked at the effective implementation of services such as transitional care, chronic care management and Medicare annual wellness visits. Beat Stine, MD, MPH , assistant dean for clinical education, 黑料网; professor; director, Medical Student Programs, Department of Family Medicine, also presented \u201cReverse co-location: Case study of integrating primary care in a mental health setting.\u201d Reed says that both presentations were well received by SAMHSA leadership as well as grantees.<\/p>\n Jeff Neer presents at North Carolina NAMI Conference<\/strong><\/p>\n Jeff Neer, <\/b>LCSWA, LCASA, CPSS , Wake ACT Team, was a panelist for “The Crisis Navigation Project: A collaborative approach to promoting psychiatric advance directives,” at the North Carolina NAMI Conference October 13 in Durham. <\/b> He discussed his participation in a recent study on the facilitation Psychiatric Advance Directives (PADs).<\/p>\n<\/div>\n","protected":false},"excerpt":{"rendered":" L-R A .Clay, R.Reed MD, and P.
