When a little more help is needed.<\/strong><\/h2>\n

The Case Management team provides community-based services for individuals with serious mental illness who need additional assistance to improve well being and to connect to helpful care and services. Working with individuals and their families, the team assesses needs and collaborates with clients to develop a plan that focuses on the client\u2019s goals.<\/p>\n
