ALLOW A 2-WEEK LEAD TIME FOR BADGE REQUESTS AND DELIVERY.<\/span><\/p>\n<p>The Planning Office provides electronic door access badges that are programmable, allowing various levels of access to all School of Medicine buildings, as well as to spaces that are specific to your role. Please refer to the current SOM Photo ID Badge Request Procedure for all requirements and delivery options (see link on left).<\/p>\n<p>For a new badge or changes to a current badge, use the Photo ID Badge Form (<a href=\"https:\/\/www.med.unc.edu\/planning\/id-badges-building-access\/photo-id-badge-request-form\/\">click here<\/a> for the most recent version).<\/p>\n<ul>\n<li>Fill out online badge request form completely.<\/li>\n<li>Form must have authorized signature from your Department.<\/li>\n<li>Have the Department head or designee email the signed form + passport quality color photo (solid neutral background, no outdoor photos or scenery) to the Photo ID Badge Office. Photos must be in JPG, PNG, or PDF format.<\/li>\n<\/ul>\n<p><span style=\"color: #ff0000\">PLEASE FILL OUT ALL FORMS COMPLETELY TO AVOID PROCESSING DELAYS.\u00a0<\/span><\/p>\n<p><b>Restricted Access<\/b>: Access to Comparative Medicine,\u00a0Irradiator Rooms, Select Agent Facilities, and other restricted areas may require additional authorizations.
