Detailed information about the <a href=\"https:\/\/www.med.unc.edu\/neurology\/new-patient-referrals-1\/adult-referrals\/\">referral process may be found <span class=\"external-link\">here<\/span><\/a><span class=\"external-link\"><span class=\"external-link\">.<\/span><\/span><\/p>\n<h3>What to Bring to the Appointment<\/h3>\n<ul>\n<li>Your insurance card<\/li>\n<li>Physician referral forms if required by insurance<\/li>\n<li>A list of current prescriptions and\/or over-the-counter medications you are taking, including dose and frequency<\/li>\n<li>Pertinent information about your medical and surgical history<\/li>\n<li>Any relevant films or records you may have<\/li>\n<\/ul>\n<h3>Insurance and Payment Information<\/h3>\n<p>ºÚÁÏÍø Health Care System is a provider for Medicare and most major insurance plans. See <a class=\"external-link\" title=\"Insurance\" href=\"http:\/\/www.uncmedicalcenter.org\/uncmc\/patients-visitors\/billing\/insurance-accepted\/\" target=\"_blank\" rel=\"noopener noreferrer\">health plans accepted<\/a> for a complete list.<\/p>\n<p>We provide insurance billing. Anything not covered by insurance will be your responsibility. We request payment at the time of your appointment for services that are not covered by insurance. Your insurance company may also require you to pay a co-payment at the time of your appointment. When necessary, our staff will work closely with patients who require a payment plan.<\/p>\n<p>If you have any questions, please call us at the phone number (984) 974-4401.
