\"\"We are a specialty practice and most patients seen in our clinic require a referral from a physician, physician\u2019s assistant, or nurse practitioner. We ask that the referring provider send relevant medical records and any lab or test results necessary for the appointment. This helps ensure that all patients who see us will benefit from the visit.<\/p>\n

How To Place a Referral<\/h2>\n

Use a faxed referral to refer a patient<\/h3>\n

Please have your physician (either primary care of specialist) fax a completed clinic referral form, along with relevant medical records (e.g., clinic notes and test results) to 984-974-5751. Upon receipt, the staff will review the request and identify the appropriate provider. Next, our scheduling team will call to set up an appointment.Should you need to check to see if an appointment has been made, please call 984-974-5706.<\/p>\n
