Patient Care Overview<\/h2>\n

The practice of Hospital Medicine is multidisciplinary and collaborative, offering patients many benefits. Our hospitalists interact with everyone on a patient\u2019s care team, including resident and consulting physicians, unit nurses, aides and coordinators, medical students, physical and occupational therapists, nutritionists, pharmacists, case managers, and social workers.<\/p>\n

Coordinated Care, During Your Hospital Stay<\/h2>\n

Hospitalist providers, along with the multidisciplinary care team, work to create comprehensive care plans for patients who are hospitalized. They provide 24\/7 attention to patient care needs, including diagnosis, treatment, and the performance of medical procedures (within their scope of practice). Hospitalists follow up on tests and adjust treatment regimens as needed, and they talk with patients and their families about their care.<\/p>\n

Because they are focused on providing efficient and effective patient care, our hospitalists are at the leading edge of changes in quality and process improvement, working to optimize use of our hospital resources.<\/p>\n

Coordinated Care, From Hospital to Home<\/h2>\n

Hospitalists work to ensure a safe transition of patient care from hospital to home or post-acute care facilities \u2013 an important ingredient for effective longitudinal care. They arrange follow-up appointments, prescribe medications on discharge and coordinate future care needs with primary care physicians. The focus of the hospitalist team is on communication: making sure both patients and their care team have detailed information about the services provided, including any new diagnoses for which treatment was initiated or changes in medications made.<\/p>\n
