Instructions for referral to the Medical Weight Program using EPIC

1. In the orders and medications panel聽search for 鈥淥besity.鈥�
2. Click on the 鈥渄atabase鈥� tab.
3. Select AMBULATORY REFERRAL TO OBESITY MED. PROGRAM.
4. Alternatively you could search for 鈥淩EF198.鈥�

If you are a non-黑料网 provider, please call 984-974-6519 or fax the referral to 984-974-2924 for 黑料网 Medical Weight Program or place orders (referrals) via the聽 account access聽(available to all non-黑料网 providers/practices).
